General Data Protection Regulation Privacy Statement
I have deemed that it is indicative of that fact that you have come to a Chiropody Appointment here, that I become aware of some details about you, which are on a record card, kept in the surgery. No details are held on computer. I do not use your details for anything else other than your treatment.
Details that I take are:
- Address and telephone number e.g. required in event of changing appointment
- Date of Birth e.g. some aspects of what you present with can be age related.
- Doctor’s surgery you attend – sometimes I do refer you to the Doctor or make an appointment for a District nurse to visit
- Medication you take – will give me a wide perspective of your general health and whether condition of your feet is related e.g. diabetes, etc.
- Allergies – elastoplast and latex gloves are used in the surgery and knowledge of such allergy would mean we have to be extra careful to ensure we do not use such products on you
- Occupation– gives me some background as to the extent you are on your feet and how it is affected by what you do, eg wearing of safety boots.
Without these details I cannot fulfil my commitment to you, which is to treat you to the best of my ability and knowledge.
Should I deem it necessary to refer you to another practitioner or Doctor, I allow you to self refer, or in event of me contacting direct, it will be with your permission.
For any case study that I use for evidence of Continuing Professional Development, I will seek your permission first, and you are not identified by name, only by case number
Any record of your treatment is kept, for a period of eight years, as is legally required by our profession. Any information we hold on you will be shredded 8 years after your last treatment.
For those of you that pay by credit or debit card, I have, each year, to declare myself compliant with the Service Provider that manages these payments, to include such matters as no name or number , except for the final 4 digits, is printed on your receipt and that all receipts are kept in locked storage.
Other than checking the payments have been received no computer data evidence is kept.
